Bring your Account Executive in on the discussion. They want to make sure you are happy, and will follow up with support. If you are on Twitter, you can try and send a message to @Salesforce. Their social media people do a pretty good job of tracking down cases as well.

From free developer edition you can not create a case. You can call basic support and log a case via phone.
You can create a case on production environment and in sandbox. I had a talk with premier support and get the confirmation.
Try calling 0016026594665, explain your case. I had the similar issue (Lost Internet connection), they were pretty supportive and rescheduled my exam. This was when I called them up immediately.
One of my colleague had a similar issue and raised a case, he got a response in about a weeks time.
My suggestion try both routes.
