I've seen that cause issues in the past for some clients. Also if you record debug logs against the user the form is running under or simply have the Developer Console open when you try to test the form and see if it any information is recorded as to why it was rejected or blocked. It could be a required field not getting populated or a validation rule is blocking the insert as well. Apr 7, 2015 at 12:47 comment added Robin De Bondt Seems I was wrong, you can't set the field through Web-to-Lead. Have you checked @SFIntegrator suggestion? Might be one of your validation rules blocks off the creation of the Lead.
