Welcome to SFSE! Please take the tour and read How to Ask. It's good that you've included your code (really hard to find issues without that). It'd also be helpful to include the code for the trigger that calls this AttachmentTriggerHandler class. It could be that you simply need to add a DML statement, but it's hard to say if you need that (or if that'd even fix your problem) without seeing the trigger too.
